Refer to Slack Intergration.

Steps:

(1) Configure a PagerDuty Actor: refer to Slack Actor

(2) Configure a PagerDuty API Configuration: refer to Slack API

(1) Configure a Behavior Action: refer to Behavior Action

What you should do is simply replacing "Slack" by "PagerDuty".
